Mobilization of the temporal pole as integrated step in microsurgical clipping of pure posteriorly directed posterior communicating artery aneurysm

چکیده

A pure posteriorly posterior communicating artery (PCoA) aneurysm represents a surgical challenge. This is mainly when there need for good exposure of the aneurysmal neck, sac, PCoA, and anterior choroidal arteries. Ruptured directed PCoA imposes significantly extra challenge as surgeon undergoes dissection through tight brain. Even with measures commonly used to attain brain relaxation like lumbar drain cisternal fenestration. Here, we describe technique temporal pole mobilization (TPM) an integrated part microsurgical clipping ruptured aneurysms. implicated in twenty-three successive cases aneurysms neurosurgery teaching hospital Baghdad, Iraq, no reported complications.

Microsurgical clipping for the true posterior communicating artery aneurysm in the distal portion of the posterior communicating artery

BACKGROUND Aneurysms arising from the posterior communicating artery (PCoA) itself are rare in which aneurysms usually located in the proximal portion of the PCoA. The authors report a case of the true PCoA ruptured aneurysm in the distal portion of the PCoA. CASE DESCRIPTION The patient was an 83-year-old man who suffered subarachnoid hemorrhage. Cerebral angiography revealed a saccular aneu...

Immediate results of microsurgical clipping of posterior communicating artery aneurysms using the pretemporal transclinoidal approach.

BACKGROUND We evaluated adverse ischemic events as early surgical results of microsurgical clipping of 44 and 34 posterior communicating artery (PComA) aneurysms through the pterional transsylvian and pretemporal transclinoidal approach, respectively, between January 2007 and October 2010. METHODS Patients undergoing PComA aneurysm clipping were divided into two groups, and their immediate su...

A case of delayed contralateral oculomotor nerve palsy after microsurgical clipping of a posterior communicating artery aneurysm

Delayed postoperative oculomotor nerve palsy is a rare complication that occurs in the setting of microsurgical aneurysm clipping. While a number of theories have been postulated to explain the development of delayed postoperative oculomotor nerve palsies, the underlying pathophysiology of such complications still remain to be elucidated. In this report, we present a case of delayed postoperati...

Fornix infarction after clipping of anterior communicating artery aneurysm.

We report a case of a 56-year-old, right-handed woman with short-term memory impairment after clipping of an incidentally discovered 6-mm anterior communicating artery aneurysm. She quit smoking five months before surgery after a 30 pack-year smoking history. Her previous medical history was otherwise notable for depression, for which she had been prescribed fluoxetine. Her father died secondar...

Posterior communicating artery aneurysm associated with duplicated posterior communicating artery and bilateral fetal posterior cerebral arteries.

Because of the location, skull-base chordomas often present with headaches and diplopia and sixth and fifth nerve palsies are the most common neurological signs.[1,2] The interval between onset of symptoms and diagnosis of clival chordoma is quite variable, 2.5-4.4 years.[4] The uncommon clinical presentations include: Nasal obstruction, nasal bleeding, and subarachnoid hemorrhage.[3,5] Spontan...
